Please familiarise yourself carefully with the terms and conditions of the educational allowance for the UCSF Fellows Training Course, listed below.
Terms and conditions
This application is intended only for an educational allowance for the UCSF Fellows Training Course on 26 October 2018. Any paperwork required to attend the workshop (eg. visa) remains the responsibility of the attendee. Once the educational allowance has been accepted by the attendee, it cannot be transferred to another recipient, unless written consent and agreement can be sought  by all parties (including the congress organisation and University of California San Francisco). If the applicant is unable to take part in UCSF Fellows Training Course, they are obliged to officially cancel their allowance as soon as possible to allow another candidate to benefit. Cancelling and failing to notify the congress organisation may lead to disqualification for any future educational allowances organised by UCSF or cme4u GmbH. The educational allowance will be deemed unused in case of applicant’s absence due to illness or other unavoidable circumstances, and will require written explanation to UCSF and cme4u GmbH. No reimbursements will be made in case of cancellation. The UCSF Fellows Training Course is not CME-accredited.

Application process
Deadline for applications for the UCSF Fellows Training Course is on September 26, 2018. Successful candidates will be chosen based on criteria listed below. Successful candidates will be notified by email after October 5. For the best chance to secure the educational allowance, please apply as early as possible, as educational allowances will be capped to manage expectation. The congress organisation reserves the right to amend or close the application deadline when seeing fit.

Eligibility
The following criteria will be considered during the selection process:
• Fellowship status
• CV
• Country of origin (a very limited number of travel stipends are available to fellows based in the US, while registration stipends are available to US as well as international fellows)
• Time of application

Incomplete applications will be disqualified automatically.

Sponsorship
Indirect financial support in the form of an educational allowance may include workshop registration and/or transport and/or accommodation. The conference organiser reserves the right to review the support needs of each application and adjust them accordingly.

Confirmation process
Successful applicants will receive their confirmation by email and are expected to officially accept their workshop place in writing within 72 hours. Fellows participating in the UCSF Training Course will be invited to attend the CSI-UCSF congress as a guest registrant. A limited number of full and partial educational allowances for participating fellows are available (see expenses policy). All educational allowances will be processed by the conference organisation. Recipients of an allowance are responsible for booking their own travel, registration and accommodation and should submit their expenses by 09 November, 2018. Reimbursement of expenses will be capped by the amount of the allowance and must comply with cme4u GmbH expenses policy. Any official declaration, such as letters of invitation for visa requirements, can be made upon request in due time.
Expenses Policy
The educational allowance for UCSF Fellows Training Course can cover some or all of the below:
• Congress registration: $200
• Hotel allowance: accommodation for one night, up to $180
• Flight: economy, up to $300  
The educational allowance will not cover any meals or other amenities, incidentals or expenses. Breakfast and lunch are served at the congress centre as part of the registration fee. Cme4u GmbH will confirm the allowance allocated to successful applicants by e-mail. Reimbursement forms will be available on 26 October, 2018 from cme4u GmbH and must be submitted by 9 November.
